Forest Park Swim Club Waiver and Release of Liability

I, the undersigned, understand and acknowledge that the use of Forest Park Swim Club and related facilities (hereinafter referred to as "FPSC") involves inherent risks, including but not limited to the risk of drowning, injury, or death. By signing this waiver, I agree to the following terms:

Assumption of Risk
I hereby acknowledge and assume all risks associated with the use of the FPSC, including but not limited to slips, falls, accidents, drowning, injuries from diving or jumping, and any other activities related to swimming and pool use.

Release and Waiver of Liability
I, on behalf of myself, my heirs, executors, administrators, and assigns, hereby release, discharge, and hold harmless Forest Park Swim Club, its owners, directors, employees, agents, and representatives from any and all claims, liabilities, damages, expenses, or causes of action, known or unknown, that may arise from or be related to my use of FPSC.

Indemnification
I agree to indemnify and hold harmless Forest Park Swim Club, its owners, directors, employees, agents, and representatives from any and all claims, liabilities, damages, or expenses, including attorney’s fees, that may arise as a result of my use of the FPSC or my breach of this waiver and release form.

Medical Consent
In the event of an emergency, I hereby consent to any medical treatment deemed necessary by emergency personnel, physicians, or other medical professionals. I understand that I will be responsible for any medical costs incurred.

Rules and Regulations
I agree to abide by all rules and regulations set forth by Forest Park Swim Club for the safe use of the FPSC. I understand that failure to comply with these rules may result in the revocation of my privileges to use the Facilities.

Photography and Video Release
I consent to the use of any photographs or video recordings taken of me during my use of the Facilities for promotional or commercial purposes by Forest Park Swim Club, without compensation.

Governing Law
This waiver and release form shall be governed by and construed in accordance with the laws of the state of California.

Acknowledgment of Understanding

I have read this waiver and release form and fully understand its terms. I understand that I am giving up substantial rights, including the right to sue. I acknowledge that I am signing this agreement freely and voluntarily and intend by my signature to be a complete and unconditional release of all liability to the greatest extent allowed by law.
